Bug with ToS quest?

Description

I have never even set foot in ToS before with this character, went today to talk to the Transmuter, he didn't say anything to me, after walking out and walking back in a couple times, he still didn't say anything. I checked my quest log and it shows the quest failed. Huh? How can I fail a quest that I never started? So now I have no chance of getting detect artifact and I don't have a nifty list of wand locations, so I'll probably never have a/b/s.

There is only one way the transmuter ignores you, and it wouldn't show up as a fail.
