Strength drop bug

I happen to have recently noticed that I'm am occassionally dropping my offhanded weapon due to strength loss, while that is perfectly normal I have noticed a few weird things Occassionlly I will not ever get a echo telling me I dropped a weapon This mostly seems to be occuring after a combat starting wither, or Impale; It doesn't however seem to matter if its PC or NPC.
